Enhancing Type 2 Diabetes Care by an Individualized and Group-Based Therapeutic Patient Education Program

Cluster randomized controlled trial evaluating an interdisciplinary type 2 diabetes (T2D) management program combining therapeutic patient education (TPE), individualized care plans, group-based quality circles, and regular PROM collection and feedback. The study aims to improve clinical outcomes, health-related quality of life, patient experience, self-management behaviors, and assess cost-effectiveness compared with standard care.
