PrimaryPROMS

PrimaryPROMS is a digital health feedback system designed to routinely collect information on patients’ health status and personal experiences via short questionnaires completed before or between primary care visits. The responses are analysed and delivered back as reports to both participating patients and general practitioners, helping guide consultations and identify health needs proactively.

Organisation: Developed by the EQUAM Foundation in collaboration with family physicians, patients, and health insurers; broadly supported by stakeholders involved in Swiss primary care quality and value-based health care.
