The Cancer Coach serves as a personal, non-medical point of contact for people with cancer and their relatives, supporting them throughout their care journey and beyond. As a dedicated contact person outside the hospital, the Cancer Coach helps patients navigate psychosocial, practical, and social challenges alongside cancer treatment.
The Cancer Coach coordinates non-medical support alongside medical treatment, connects patients and families with appropriate services, and facilitates collaboration between healthcare and social care providers. By identifying individual needs early and ensuring seamless coordination, the project aims to create a more integrated, patient-centred care pathway and improve quality of life.
The pilot project also evaluates the impact of this new model to support its continuous improvement and long-term implementation in outpatient oncology care.
