Alliance DT2: Rethinking Type 2 Diabetes Care Through Therapeutic Patient Education

In this Lunch & Learn session, Ksenia Tugay, Dr. David Ehlig, and Carla Walker present the Alliance DT2 project, a collaboration between Réseau Delta, Groupe Mutuel, and the University of St. Gallen exploring new approaches to type 2 diabetes care in Switzerland.
The session will provide insights into an interprofessional and personalised patient education programme for people with early-stage type 2 diabetes, including the role of therapeutic patient education, PROMs, and collaboration between healthcare professionals.
Speakers will also share early experiences and reflections from the ongoing project and discuss practical challenges and opportunities related to chronic care management and Value-Based Health Care (VBHC).
